The District will be switching dental insurance providers effective January 1, 2019. Our new dental insurance broker will be Choice Plans Inc., and the new dental insurance carrier will be Guardian. ID cards will be mailed out shortly after the policy starts on 1/1/2019.

Q: Who is the new dental insurance carrier and when do they start?
A: Guardian will be the new insurance carrier beginning January 1, 2019 with group #554377
Q: What do I need to do in order to be enrolled?
A: Nothing, you and any current dependents on your plan will automatically be enrolled in the new
Guardian policy. You only need to complete an enrollment form if you want to make any changes.
Q: What do I need to tell my dentist?
A: Please inform your dentist that your dental insurance carrier will change to Guardian beginning
January 1, 2019. You will get an ID card mailed to your address that you can give to your dentist. If you
have a dental claim denied for a date of service after 1/1/19 please verify that your dentist has your new
insurance information. Ameritas will correctly deny all claims after 1/1/19
Q: How are my dental benefits changing?
A: Your dental benefits are not changing in any way. The benefits under Guardian are matching the
benefits you previously had under Ameritas.
Q: What if I have a dental appointment scheduled for after 1/1/19?
A: You have full benefits and can be seen beginning 1/1/19 under the new Guardian policy. Please
inform your dentist’s office about the insurance change so they can bill the correct carrier.
Q: If I currently have work in progress, how will it be handled? (Ex. I have a temporary crown on my
tooth right now and am scheduled to get a permanent crown after 1/1/19)
A: As long as you were covered by the prior plan, work in progress will be covered under the new
policy. If you had a mold taken prior to 12/31/18 it will be paid under Ameritas even if additional work
for the same procedure is done after 1/1/19. If the mold will not be taken until after 1/1/19 the
remaining claims should be sent to Guardian.
Q: How will my deductible and annual maximum be affected by this change?
A: Both your deductible and annual maximum reset on January 1st each year under your current policy.
This will continue with Guardian and you will have a new deductible and annual maximum starting
1/1/19.
Q: Will I get a new dental ID card?
A: Yes, you will get an ID card from Guardian. They will be delivered to your home address shortly after
the policy starts on 1/1/19. If you have an appointment before you receive your ID card your dentist can
verify coverage using your group number #554377 and your social security number.
Q: How do I know if my dentist is in the Guardian dental network?
A: You can check the participating provider list on GuardianLife.com
- Go to GuardianLife.com and click on “Find a Provider” at the very top of the website then click
on “Search Providers” in the green box on the following page.
- On the next page the “find a dentist” tab should already be selected. Under that you need to
check off “PPO” under plan type then enter the zip code of the area you want to search in. Once
complete you can click on the “Search” button.
- After clicking search a list of dentists will populate below. Every dentist that shows up is in the
Guardian network.
- At this point you can also use the filters on the left side of the website to search for specialists.
Q: Can I check my dental information online?
A: Yes, you can make an online account on GuardianAnytime.com and have access to all your dental
information including EOB’s and dental history. You may also order a new ID card to be sent to your
address. You will have to wait until the effective date before being able to make an online account.
Q: To what age is my dependent covered?
A: Dependents are covered up to age 19 or up to age 26 if they are a full-time student. If your
dependent is a full-time student you will need to provide proof of student status once a semester in
order to continue coverage. Proof of student status can be either a letter from the registrar’s office
listing the semesters the student is enrolled or a copy of the student’s class schedule. If you submit a
class schedule it must include the student’s name, the school’s name, and what semester the classes are
for.

A: If you are having any type of dental problem and do not know where to get an answer, please call
Ryan Stewart at (800) 559-5232. He will personally work with you and get an answer to any dental
questions you may have.
